Job Description

Community Outreach Liaison/Coordinator

Mutual Assistance Network (MAN) is a community development corporation dedicated to the enhancing the social and economic infrastructure of Northern Sacramento. MAN is hiring one full time Community Outreach Liaison (COL) for the Black Infant Health (BIH) program. Black Infant Health is a specialized program that helps address and improve maternal and women's health. It is a participant centered, strength-based group intervention with complementary case management that embraces the life course perspective and promotes skill building, stress reduction and life goal setting.

Hallmarks of the BIH program:

1. Improve African-American infant and maternal health.

2. Increase the ability of African-American women to manage chronic stress.

3. Decrease Black-White health disparities and social inequities for women and infants.

4. Engage the community to support African-American families' health and well-being with education and outreach efforts.

Position Title:

Community Outreach Liaison

Position Summary: The Community Outreach Liaison shall serve as the primary contact with external agencies and organizations in order to promote referrals to the program and maintain community relationships. The COL is the also the primary contact for all potential BIH participants, responsible for developing a quality, trusted relationship with the participants and their families to introduce them to the BIH program and support them in the enrollment process. Position is full-time, hourly and non-exempt.

Compensation: $22-$24/Hour

The Community Outreach Liaison reports to the Program Manager.

Essential Functions:

1. Outreach Activities

Develops and maintains a site-specific Recruitment Plan for BIH, with the Program Manager

Establishes a database of community agencies and creates relationships to obtain BIH referrals

Maintains relationships with medical and community service providers who are the primary referral sources into BIH;

o Develops Partnership Agreements to assist in providing referrals to the BIH Program

o Conducts outreach activities on a regular basis; for example, conducting in- service trainings and BIH Orientations for Partnership Organizations;

o Attends inter-agency and community meetings

Schedules outreach activities for the BIH team to attend and works with manager to assign staff

Builds relationships with local community organizations where BIH groups can be held, and is responsible for the ongoing scheduling of those group dates and times

2. BIH Program Support

Maintains schedule of Child Care coverage needed for BIH groups, and works with Program Manager to ensure staff are assigned

Coordinates and schedules door-to-door transportation needs for participants to/from group

Plans group meal schedule and oversees ordering of healthy, nutritious meals for each group session for participants and children attending Child Care

Support with group facilitation as needed

Support with Child Care coverage as needed

3. Supervision, Multi-Disciplinary Resource Team Case Reviews, Training

Attend weekly supervision with the Program Manager.

Participate as a member of the Black Child Legacy Multi-Disciplinary Resource Team (MDT) of service providers.

Consult with MDT members regarding issues the family may experience that need additional assessments, referrals, or services, and facilitate referrals to BIH programs.

Attend required trainings provided by the County of Sacramento and its Contractors.

Attend other trainings as suggested by Program Manager.

4. Case File Documentation

Complete all documentation as required by BIH Policies & Procedures in the timeframe required.

Maintain confidentiality/privacy of family information in case file documentation.

5. Other Functions

Maintain a safe, clean, resourceful, and enriching environment for families and staff. Assist with and participate in the Family Resource Center (FRC) as needed.

Other duties as assigned

Knowledge, Skills and Abilities:

Must be able to read, write, speak and understand the English language.

Knowledgeable about local community resources.

Ability to serve as a catalyst for change by modeling and mentoring behavioral change, which can break down bias, prejudice, and other institutional barriers that exist.

Baseline understanding of African American maternal and infant health disparities, especially related to historical and systemic issues on both a national and local level

Act as a communication and trust bridge between the family and systems.

Ability to establish and maintain personal and programmatic boundaries while providing supportive services.

Experience in working with culturally diverse communities and families, with the ability to be culturally sensitive and appropriate.

Demonstrate good writing and organizational skills

Skill to maintain a professional, confidential work environment.

Ability to manage multiple tasks in an efficient manner.

Knowledge of child abuse issues, substance abuse and family dynamics

Knowledge of child development and behavior.

Ability to work evenings and weekends, as required.

Ability to communicate clearly both orally and in writing.

1-3 years experience in social services in community settings with families preferred.

Able to deal with stressful situations.

Other Qualifications:

Must be at least 18 years of age.

Must be a U.S. citizen or lawful permanent resident.

Clearance of fingerprint background check.

Bachelor’s degree in social services or related field

Must possess a valid California Driver's license, reliable transportation and auto insurance.

Able to travel between sites and to offsite events.
