Our agency is looking for a Case Coordinator to join our growing team. Our Case Coordinator is an individual who is caring, helpful, compassionate and have experience working for disabled individuals in a supported living as well as an independent living facility. The Case Coordinator will need to have effective written and oral communication skills, have good interpersonal, organizational and time management skills. This position is Monday through Friday, from 8am-4:30pm. If you are interested, please send your resume.
Responsibilities
Perform client’s assessment, quarterly and annual reports.
Schedules and conducts meetings and appointments.
Supervise staff and scheduling.
Monitors and records goals and objectives with clients.
Performs other duties as assigned.
Skills and Qualifications
Reliable and Dependable
Previous experience in a healthcare, or home care environment (preferred)
A minimum of one year of professional experience
Valid Driver's License
Eligible to work in the United States
CRIMINAL BACKGROUND CLEARANCE REQUIRED
