The Poway Adult Day Health Care Center is a day program aimed towards providing activities, medical oversight, social services, and daily care to older adults with dementia, Alzheimer's, mood disorders, and other chronic healthcare needs.
Job Overview
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our healthcare team. The ideal candidate will be responsible for providing high-quality nursing care to participants on a daily basis. This role requires excellent organization skills, communication, patience, and compassion for working with older adults with various disabilities, mental health conditions, and cognitive impairments.
For nurses with experience working in a SNF setting, this offers an attractive alternative, program hours are daytime only, No evening or weekends. No on call. Closed on major holidays.
Shift Hours Mon-Fri: 8:30am-3:30pm
Pay rate dependent on experience and qualifications.
Duties
Administer prescribed and authorized daytime medications
Assess and record blood pressure, blood sugar, and weights on a regular basis
Assess for emergent health care concerns requiring follow up with appropriate providers/family
Charting and documentation of treatment plans on a daily basis
Follow up with families regarding health related concerns/questions
Collaborate, communicate, and assist other staff with coordination of care for participants
Assist with special diet preparation at meals
Communicate with participant's health care providers
Ensure compliance with healthcare standards and regulations.
Qualifications
Current and valid Licensed Vocational Nurse (LVN) license or Registered Nurse (RN) License
Experience in adult day care, senior care, caregiving, dementia care, geriatrics or nursing homes is preferred.
Strong communication skills and the ability to work collaboratively within a team.
Commitment to maintaining high standards of patient care.
