Position Summary:
As a Home Health RN, you will be responsible for the comprehensive management of
patients in their homes. Your role includes direct care, coordination of services, and
development of safe, individualized care plans using OASIS (Outcome and Assessment
Information Set) and current best practices.
You’ll provide compassionate, skilled care for adult patients (ages 21 to 101) with varying
medical needs and home environments—making a real difference in their daily lives.
Responsibilities Include:
● Delivering high-quality, individualized nursing care in patients’ homes
● Conducting comprehensive assessments , initiating care plans, and setting appropriate
goals
● Managing patient care visits and establishing safe frequencies and durations
● Performing clinical tasks including IV insertion , catheter care , tube feedings , and
BLS/CPR as needed
● Collaborating with healthcare teams, families, and caregivers to ensure continuity of care
● Educating patients and families on condition management and recovery
● Traveling to patient homes throughout your chosen region
● Accurately documenting all care and communications in the EHR system
✅ Qualifications:
● Graduate of an accredited school of nursing
● Current California RN license unrestricted and in good standing
● BLS (CPR) Healthcare Provider certification
● Preferable 1 year of recent experience in home health , acute care , or
medical-surgical nursing
● Familiarity with OASIS documentation preferred
● Must have or be willing to obtain professional liability insurance
● Valid California driver’s license , vehicle registration, and auto insurance
● Reliable vehicle and willingness to drive to patient homes
● Able to provide a pre-employment health clearance , negative TB test , CXR , and
required vaccinations per California health regulations
Additional Information:
● Mileage reimbursed at the current California IRS rate
● Flexible employment options: Part-time, or Per Diem
● Work Location: On the road (patient homes throughout San Diego County)
