Job Summary
We are looking for a patient, compassionate Nurse Practitioner (NP) to visit home-bound seniors or the mentally-ill in group homes. The patients will largely stay the same and be assigned to you. You get to visit them routinely and be available, at times, for on-call visits (seldom). We are expanding in the East County area. We also have a group in Vista for you. We are looking for a long-term commitment.
Our services include:
Assist in managing chronic ill conditions
Most of the time, you are back up to their PCP who doesnât see them
Coughs, colds, sore throats, ear infections
Fever or flu-like symptoms
Sprains and strains
Allergic reactions, asthma, rashes
Responsibilities:
To get to know each patient and work with them on their terms. Most of our hpi is done by observation.
To be consistent with the patients, administrators of the homes, and work well with the practice manager. Communicate timely by text or phone. Keep concise abbreviated notes.
Be flexible with what medications are prescribed to the patient based upon what is covered by their insurance (most are on Medicare and have a fixed income)
Decide when or if a patient gets to have non-emergency transportation to the UC or ER.
Some (seldom) 24 - 48 hour coverage may be needed
Experience Requirements:
Medicare (PECOS) provider (or the ability to become so immediately)
Ability to discern changes â observed, stated, or reported.
Ability to assist in the management of multiple medications for chronic illnesses.
Good bedside manner with the elderly
Active and current medical license
Active and current DEA number
NOTE: We do NOT prescribe scheduled pain meds for anyone.
Job Type: Part-time. Independent Contractor. Very good compensation.
