As a Speech Therapist you will:
Complete initial and on-going assessments and evaluation to identify the clientâs level of functioning and ability to communicate, including OASIS assessment at appropriate time points.
Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.
Develop the plan of care in consultation with the physician and other care team members.
Observe, record and report to the physician and/or Clinical Supervisor the patient's reaction to treatment and any changes in the patient's condition.
Evaluate the home environment and make the appropriate recommendations.
Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program.
Participate in in-service education, utilization review and team conferences.
Provide discharge planning, as appropriate.
Evaluate effectiveness and outcomes of care / response to therapy interventions.
Prepare and submit, as directed, all case documentation, including evaluation data, treatments, clinical and progress notes and required payroll information according to Agency policy.
Provide services within the Agency's catchment area.

Qualifications:
Current, active license as a Speech Pathologist by the California Board of Medical Quality Assurance.
Core competency per skills checklist.
At least one year of experience in speech therapy.
