ENHANCED CARE MANAGEMENT (ECM) COORDINATOR

SUMMARY: The Enhanced Care Management Coordinator will be responsible for overseeing and coordinating care management services for individuals with complex medical and behavioral health needs. This role will involve working closely with healthcare providers, insurance companies, and other members of the care team to ensure the delivery of high-quality, comprehensive care. The Enhanced Care Management Coordinator will also collaborate with interdisciplinary teams to develop and implement care plans that meet the unique needs of everyone.

Qualifications:

· Bachelor's degree, preferably a health sciences background.

· Knowledge of medical and behavioral health systems, including insurance coverage and reimbursement processes.

· Excellent interpersonal and communication skills, with the ability to collaborate effectively with healthcare professionals, patients, and families.

· Proficiency in using electronic medical/health records systems and other relevant software applications.

· Knowledge of care management principles, including assessment, planning, implementation, and evaluation.

· Ability to work independently and manage multiple priorities in a fast-paced environment.

· Previous experience in care coordination or case management, not required.

Responsibilities:

· Coordinate and monitor the delivery of healthcare services, ensuring adherence to care plans and quality standards.

· Facilitate communication and collaboration between healthcare providers, insurance companies, and other members of the healthcare team.

· Provide education and support to individuals and their families regarding their care plans and available resources.

· Stay updated on industry best practices and guidelines related to care management.

· Collaborate with community organizations and agencies to enhance care coordination services.

· Participate in ongoing quality improvement initiatives to ensure the delivery of high-quality care.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Qualifications:

- Must have a Bachelor’s degree, health sciences preferred.

-Bilingual: English and Spanish preferred (not required).

Experience:

Minimum of 0-1 years of coordinator experience in health care/health services, with at least 1 year of experience in Medi-Cal managed care and/or as a Medical Assistant. Experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions and/or residing in long-term care is preferred but not required.

Desired Skills and Abilities:

- Highly skilled interpersonally, with excellent relationship skills.

- Able to sufficiently engage with members and healthcare providers in a variety of settings such as on the phone, at member’s homes, streets if homeless, in hospitals and other health settings.

- Ability to develop relationships with community members and leaders.

- A high degree of skillful decision-making and judgment, in an autonomous position, including knowing when to consult with the team, supervisors, and experts.

- Understanding of and sensitivity to mental health conditions, substance use disorders, and multi- cultural communities.

- Awareness of the impact of unmitigated bias, and judgment on health; a commitment to addressing both.

Physical Demands:

May be required to lift objects weighing up to 25 pounds. Sitting in normal seated position for extended periods. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity is required to manipulate objects with fingers rather than whole hand(s) or arm(s), for example using a keyboard.

Salary Ranges: $30 - $32/hr
