The Position

The Project Coordinator is responsible for the management and program coordination of the California Accountable Community for Health Initiative (CACHI) grant. This includes the successful creation and implementation of a local Accountable Community for Health in Santa Barbara County supporting caregivers.

The goal of the CACHI grant is “building stronger, healthier, more equitable communities.” For decades, long-standing inequities have exacerbated health issues in low-income communities and communities of color, resulting in persistent health disparities. Accountable Communities for Health (ACHs) provide a powerful framework for tackling those inequities by breaking down barriers and promoting a new way of working together in support of a target population; in this case, family caregivers.

The Project Coordinator will be expected to liaison with FSA staff working on the ACH, community partner agencies and CACHI staff; oversee project quality controls; assist with project budget management; ensure contractual targets are met; comfortable with policy and systems level work, and completion of qualitative and quantitative reports in a timely and accurate manner. This position requires a person who is an independent worker, self -motivated, enthusiastic and able to engage with people of diverse backgrounds in an easy, genuine manner. This position reports to the Senior Services Director.  This position can be based out of any of FSA’s offices in Santa Maria, Lompoc, or Santa Barbara. Some travel within SB County may be required.

Sample Job Duties and Responsibilities

Program Development, Compliance, and Evaluation

1. Oversee the CACHI grant. 

2. Research and develop the co-creation of an Accountable Community for Health (ACH). 

3. Apply the CACHI framework for accountable communities for health to existing collaborative efforts of CTSBC in support of Caregivers for Santa Barbara County.  

4. Oversee quality controls for the project, including monitoring project outcomes, evaluating project effectiveness, and staying current with research in the field. 

5. Ensure contractual expectations and targets are met and that services are within scope of practice. 

6. Coordinate collection of evaluation data, prepare reports, and ensure timely report submittal. 

7. Maintain oversight of expenditures to ensure project-related costs are within the outlined budget. Coordinate budget modification requests, if necessary. 

8. Plan and conduct project meetings, as needed. Convene subcontractor meetings with partner agencies as necessary. 

9. Plan and implement Caregiver Health Equity Forum during the final six months of the grant period.

Community Partnerships

1. Work with FSA, partner agencies, caregivers and seniors, and CACHI staff members for successful project implementation. 

2. Represent FSA and CACHI at grant project at meetings, community events, and conferences.

Management Team Member

1. Participate in Program Management Team meetings to ensure alignment in program operations and strategic program development.

2. Participate in the ongoing development, evaluation, and implementation of operational and strategic plans.

3. Implement quality assurance and risk management processes.

4. Cultivate a climate that invites and motivates employees from diverse backgrounds, providing the platform and support to grow and lead at FSA.

Employment Requirements:

1. Bachelor’s degree required; at least 5 years of relevant experience in grant coordination and management accepted in lieu of a Bachelor’s degree.

2. Bilingual in English and Spanish preferred.

3. Non-profit experience preferred, 2 years.

4. Experience with budget operations and oversight, and data tracking and analysis.

5. Experience working with community agencies, and community members.

6. Computer and internet literacy and excellent verbal and written communication skills are required.

7. Proficient in Microsoft Office, and able to adapt to using various types of software.

8. Valid automobile insurance and a valid California Driver License are required.

9. Reliable transportation is necessary to travel throughout Santa Barbara County. 

10. DOJ Background check and TB test clearance required. 

Classification, Hours and Pay Rate:

This position is full-time, salaried/exempt. Benefits include: medical (Choice of HMO and PPO offered), dental, vision, life insurance, flexible spending accounts, and a 403(b) retirement plan with an annual company contribution. Generous vacation, holiday, and sick leave also offered.

The pay range for this position is $30.00-$35.00 per hour.
