New Beginnings – NP, RN, or PHN - 40 hours per week/full-time, non-exempt in Santa Barbara County

The NP, RN, or PHN will provide care and coordinate services for clients in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses. The NP, RN, or PHN will manage homeless and formerly homeless clients who will be facing specific challenges to obtaining healthcare that often have complex, acute and chronic physical and mental health presentations.

DESCRIPTION OF ASSIGNED DUTIES AND RESPONSIBILITIES:

Screen and assess for health, psycho-social issues, mental health, and substance use disorders to determine treatment needs.

Provide client education about healthcare, including diabetes, heart disease, HIV, AIDS, mental illness, etc.

Support and closely coordinate healthcare with the client’s primary care provider and when appropriate, other treatment providers.

Case management: Managing ongoing client injuries, illness, or wellness commitments, building relationships with clients to see their health and lifestyle holistically.

Use evidence-based medicine and national guidelines including Housing First to achieve optimal population health and effective disease management.

Monitor clients (in person, in the home, on the phone) for changes in symptoms and treatment side effects or complications

Work closely with staff to identify, analyze, and coordinate internal and external resources to resolve care issues and meet the needs of each client.

Collaborate with other community agencies and consult with physicians, other staff, and/or other healthcare professionals, and refer to in-house or outside health professionals, resources, or community agencies as indicated to ensure the best possible outcomes.

Coordinate services with other organizations and programs to ensure such services are complementary and comprehensive; directing activities to maximize effectiveness and continuity of care.

Record and enter relevant information, all interactions, and services with and on behalf of the client into CenCal, HMIS/other digital platforms within timelines set forth by agency policies and procedures.

Analyze and interpret client health information/data including results of laboratory testing as needed.

Provide ongoing education and support as needed to the client and family members as applicable.

Facilitate internal or outside referrals to evidence-based health & mental health services as clinically indicated.

Adhere to ethical principles about confidentiality, informed consent, compliance, and relevant laws and all agency policies and procedures.

Maintain client privacy by adhering to HIPAA Privacy Practices

Responsible for attending all assigned internal and external meetings, ensuring timely completion of training and accurate completion of timesheets.

Perform all related duties as assigned by program management.

Understand the different roles within the interdisciplinary team and act with professional boundaries.

Demonstrate genuine care and understanding for individuals experiencing homelessness.

Health care advocacy to ensure the very best care and communication, coordination of appointments, medication, and treatments.

QUALIFICATIONS AND REQUIREMENTS:

Possession of a valid license to practice as a professional registered nurse in the State of California AND Completion of the requirements for NP, RN or PHN as specified in the regulations of the Board of Registered Nursing, California Administrative Code, Title 16, Chapter 14, Article 8, Section 1482.

Must possess and maintain a valid California license as a Registered Nurse and a BRN Certificate evidencing that the individual is qualified to use the title of Nurse Practitioner, RN or PHN, as required by the California Board of Registered Nursing

Nurse Practitioner applicants must possess a master’s degree in nursing, or a master’s degree in a clinical field related to nursing, or a graduate degree in nursing.

NP, RN, or PHN must possess an academic degree or certificate from an approved college or university-based nursing program or possess equivalent training and experience as outlined by the California Board of Registered Nursing (BRN)

Evidence of required degree(s) or certification(s) will be required at the time of hire.

Satisfactorily complete a nurse practitioner, RN, or PHN program.

Satisfactory completion of a background check (including a criminal records check) is required for employment.

Maintain credentials by keeping all required licenses current, including but not limited to California Medical License, DEA licenses, CME hours, CPR Certifications, and any other required licenses. It is the responsibility of the NP, RN, or PHN to give Human Resources current credentials before old credentials expire.

Liability insurance

Minimum of one year of paid experience in an acute care setting

Comprehensive knowledge of advanced nursing care principles, methods, and procedures

Working knowledge of medications, their actions, use, and side effects

The highest standards of ethical conduct

Provide excellent care to our clients.

Work hours are Monday – Friday, 9-5 although there will be some nighttime and weekend hours on occasion to attend community events, meet with clients, or attend to emergent client and administrative needs.

Salary is $100,000 - $110,000 annually DOE and credentials with medical and dental benefits provided, 403b contribution and matching, as well as generous paid time off. The position begins ASAP.
