Harmony Homes For Healing in Fairfield, CA is a Housing Services Provider that works with housing insecure veterans, mothers with babies, domestic violence survivors and adults in recovery. Our agency is growing by leaps and bounds and is looking for one part-time Program Intake Coordinator to join our 9 person strong team. This position has tremendous opportunity for growth and will convert to full-time after successful completion of a 90-Day probation period. Our ideal candidate is highly skilled in administrative tasks, detail oriented and a self-starter that takes pride in their work and is motivated to succeed!JOB DESCRIPTIONThe Intake Coordinator is responsible for managing all aspects of the program intake process including communicating directly with potential program participants and referral partners, collecting necessary intake information and conducting telephone interviews, processing intake paperwork, establishing and maintaining positive relationships with existing and potential program participants, responding to participant requests and concerns, and facilitating and managing Medi-Cal verification and authorization processes.REPORTS TOExecutive DirectorGeneral Duties: Manage all daily participant referral and intake operations including the establishment and implementation of intake policies and procedures. Ensure compliance with all state, federal, and regulatory requirements. Direct the implementation of improved work methods and procedures to ensure participants are admitted in accordance with policy. Maintain positive working relationships with current and potential referral sources. Ensure maximum third-party reimbursement by verifying and documenting Medi-Cal eligibility and authorization processes. Gather, collate, and report referral statistics including key customer referral trends. Maintain comprehensive working knowledge of community resources and assists new referrals and program participants in accessing social service resources. Essential Responsibilities: Welcome and greet all new residents within 24-48 hours of entry. Complete and document telephone interviews for new referrals. Schedule in-person intake interviews, move-in appointments and routine program participant check-in appointments. Update, verify and document Medi-Cal eligibility and service authorizations. Manage the processing of new service referral requests, intake paperwork, release of information forms and eligibility documents. Routinely monitor documentation and ensure adherence to HH4H documentation standards, maintain daily log and other HH4H related recordkeeping systems. Requirements: Healthy boundaries are a MUST. High school diploma/GED required, with preference given to candidates pursuing or holding a BS in a related field. Residential experience and/or direct experience in providing services to persons/families experiencing homelessness, trauma, and/or substance use preferred. Personal qualities and skills: good interpersonal and communication skills orally and in writing, initiative, positive attitude, non-judgmental, empathetic, compassionate, accountable, resourceful, good time management and organizational ability, good judgment, attention to detail, ability to manage multiple tasks and crises and prioritize, ability to work with a diverse population, and ability to work independently and as part of a team. Preferably having 1-2 years of experience in healthcare or the social services field Reliable attendance and punctual reporting to work is required. Previous training in crisis prevention/intervention strongly desired (i.e., CPI or equivalent). Valid driver’s license preferred.
