LVN or RN needed as a Clinical Case Manager for our Santa Rosa Home Care Agency. Fulltime position during office hours of Monday through Friday 8:30-5:30. We are locally owned by a nurse and our Agency does not accept Medicare.
Our Agency is busy and we need another member to join our clinical team. Our clients are located in Sonoma County only. This is not a remote position. Must supervise our caregivers in our clients' homes, open new cases and provide skilled services and Case Management. (no experience in Case Management willing to learn we will teach and mentor you) Full benefit package offered which includes mileage, health, dental, 3 weeks PTO and Company credit card for expenses.
