Job Overview

Palladia Health is attractive startup that offers the flexibility and options for stocks ownership, profit sharing and board of directors membership. Most of our patients are VIP, high end clientele that we would like to provide with continuity of care by offering outpatient therapy follow up with familiar providers. We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, hospital, home health, and community care environments. This role involves developing and implementing effective treatment plans to improve mobility, reduce pain, and enhance overall quality of life for diverse patient populations. A strong foundation in physiology, anatomy, and medical terminology is essential to deliver high-quality physiotherapy services tailored to individual needs.

Duties:

- Conduct initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.

- Supervises Therapy Assistants and Home Health Aides as appropriate

- Develop and update the plan of care in consultation with the physician and other care team members.

- Provide treatments and evaluation of equipment needs as applicable.

- Advise and consults with the family and other agency personnel as applicable.

- Develop clinical and progress notes

- Evaluate care outcomes

- Participate in in-services, staff meetings, case conferences, educational seminars, and other training programs.

- Create discharge planning

- Coordinate with Supervisor/Case Manager/Physician additional modalities and services when required.

- Collaborate on problem resolution strategies with Supervisor when problems or questions occur in the field, including charting guidelines, coordination of disciplines, and supervision of home health aide services.

- Adhere to the infection control protocols per OSHA guidelines and Agency policies.

- Ensure compliance with the Medicare Conditions of Participation and any other regulatory requirements.

- Maintain professional appearance, treat others with care and respect and demonstrate excellent customer service.

- Must have current unrestricted professional and driver license, current CPR certification and Tuberculin testing.

Skills

- Extensive knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice

- Proven experience in discharge planning, care plans, and managing diverse patient populations including pediatrics, geriatrics, and orthopedic cases

- Strong background in home & community care, hospital experience, or outpatient clinics with a focus on patient care

- Proficiency with WellSky systems for documentation and patient management

- Skilled in various physiotherapy techniques including trigger point therapy and orthopedics treatments

- Ability to work effectively in fast-paced environments such as inpatient, outpatient, or home health settings

- Excellent communication skills for educating patients and collaborating with healthcare teams
