Program Manager/Counselor

LifeLong Medical Care is looking for a Program Manager/Counselor at our TRUST Health Center in Oakland. The Program Manager/Counselor will oversee all aspects of the SAMHSA MAT program.

Benefits

COVID-19 Vaccine Policy

In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Unless a reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.

Responsibilities

Oversight of integration of mental health, substance use, and MAT services for the SAMHSA MAT program

Responsible for administrative supervision of SAMHSA MAT program staff, including hiring, training, performance evaluations, approving timesheets and schedules, and personnel issues per agency guidelines.

Provide direct SUD services, including co-occurring counseling services to clients receiving MAT; some services will be providing at the outreach sites, including homeless encampments (approximately 50% of the time)

Increase and support the retention of patients with co-occurring and SUD disorders receiving care from Street Medicine and the Trust Clinic.

Works with Trust site leadership and LifeLong MAT/Recovery Support and Reentry Program Manager to improve coordination of SUD care and services available within Trust Clinic and Street Medicine (with primary care and behavioral health care)

Performs needs assessments and coordinates with community partners to refer patients to appropriate outside resources.

In collaboration with the Evaluator/Research Manager, ensure that all grant reporting requirements are met.

Coordinate care with, and collaborate with agency-wide MAT and Recovery Support programs across LifeLong to ensure quality of care and standardization

Communicate tasks, timelines, goals, and other necessary updates to SAMHSA MAT program staff

Utilize and prepare data in collaboration with the Evaluator to inform decisions that impact future direction of SAMHSA MAT program

Ensure that grant deliverables are met

Elicit and provide feedback to program stakeholders in collaboration with the Evaluator

Assess program performance and complete PDSA cycles for continuous quality improvement.

Participates in all required supervision, team meetings and training, regular case conferences, meetings with grant project officer, site-based meetings, and appropriate agency-wide meetings.

Performs other duties as assigned by supervisor.

Qualifications

Must be registered or certified as an SUD Counselor or ASW.

Knowledge and experience providing substance use and mental health treatment services to diverse, low-income populations.

Managerial experience preferred

Demonstrated ability and sensitivity to working with a diverse staff and client population, including individuals with mental illness and substance use related issues

Excellent judgement skills and decision making abilities

Highly proficient in MS Office (including Word, and Outlook)

Ability to work effectively and calmly under pressure in a positive, friendly manner.

Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.

Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.

Must have reliable access to transportation

Must be able to abide by all conditions of employment set forth by federal and state laws regulating healthcare settings and workplace behavior and including strict confidentiality.

Experience with Epic Electronic Health Record preferred

Bilingual Spanish/English preferred
