We are currently seeking a Quality Assurance (QA) Coordinator to join our growing team. This part-time position plays a vital role in ensuring our nursing documentation meets all California Department of Public Health (CDPH), Title 22, and agency compliance standards.
Position Summary
The QA Coordinator is responsible for reviewing, auditing, and monitoring clinical documentation submitted by the nursing staff to ensure accuracy, completeness, and compliance with regulatory and agency standards. This role works closely with the Director of Patient Care Services (DPCS) and field clinicians to maintain the highest standards of quality, patient safety, and documentation integrity. This position reports to the Director of Patient Care Services and Agency Director.
Key Responsibilities
Review all nursing charts and visit notes for completeness, accuracy, and compliance with CDPH regulations, Title 22, Medicare CoPs, and agency policies.
Verify that all documentation supports the plan of care and meets state and federal home health requirements.
Identify charting errors, omissions, and inconsistencies, and follow up with nurses for timely correction.
Track QA trends, prepare summary reports, and recommend process improvements to management.
Ensure a 48-hour turnaround for reviewing clinical documentation
Collaborate with the DPCS and clinical staff to provide education and feedback on documentation standards.
Assist in maintaining agency readiness for surveys, audits, and accreditation reviews.
Ensure all electronic health record (EHR) entries are properly signed, dated, and submitted within required timeframes.
Participate in case conferences and QA meetings as needed.
Qualifications
Required:Registered Nurse (RN) in the State of California.
Preferred: At least 2 years of experience in a home health or healthcare QA/clinical documentation review role.
Strong knowledge of CDPH home health regulations and documentation standards.
Proficient in EMR systems (e.g., Kinnser, Medsys, or similar).
Excellent attention to detail, organization, and communication skills.
Ability to work independently and collaboratively in a fast-paced environment.
