The Department of Health Services has multiple openings across many different programs. Some of the responsibilities of a Senior Client Support Specialist within these programs are:
Providing outreach, case management, and resource connections to those in need
Collaborating with County programs, external agencies, and team members to gather information for client support
Conducting regular client visits, providing referrals, and assisting with transportation as needed
Documenting client information and administering crisis assessments, de-escalation, interventions, safety plans, and warm handoffs
Engaging in treatment planning, rehabilitation services, coping skills education, and rapport-building
Conducting reassessments of client needs and facilitating connections with healthcare professionals
As an ideal candidate, you are passionate about working with underserved populations and can switch gears quickly in a fast-paced environment. In addition, you possess many of the following:
Working knowledge of local community resources and services
Experience in outreach, interviewing, case management, and documentation
The ability to work in close collaboration with a multidisciplinary team
Previous experience working with clients from a variety of ages and backgrounds
An understanding of the principles and techniques of crisis management
A desire to help clients and meet them where they are in their progress
The ability to learn new computer systems
A desire to be on a supportive team and contribute to that culture
Please note: Some positions are within 24/7 programs and will need to be staffed all hours, including evenings, overnights, weekends, and holidays. Many of these positions perform work in the field, meeting clients in public spaces, as well as their homes.
