Hiring Part-time or full-time candidate
Serving East Bay Region including- Oakland, Vallejo, Berkeley, Antioch, Union City and surrounding areas
General Summary:
As a Direct support provider, your role is pivotal in fostering the healthy growth and development of individuals with Developmental Disabilities and their dependents. Your work, which includes education, modeling, coaching, and guidance, directly enhances their life skills (e.g., parenting, legal, organizational, mental health, finance), physical skills (e.g., exercise, hygiene, medical/dental visits), and social-emotional functioning. Your efforts are instrumental in improving their overall well-being and quality of life.
Experience and skills:
Six months of experience working with persons with developmental disabilities and/or intellectual disabilities or in the human services field
Fundamental math and budgetary skills and ability to assist individuals with managing a household budget
Effectively interact with people across multiple cultures
Comfort working in consumer’s homes
Ability and willingness to transport consumer's in personal vehicle
Receptive and responsive to coaching and feedback
Excellent written and verbal communication
Knowledge of Basic Technology including Microsoft Applications and Google
Qualifications:
High school diploma or equivalent
Must be age 21 or over
Ability to pass:
Criminal background check
DMV background check
Pre-employment physical
TB test
Ability to lift 50 pounds and assist individuals with transfers to and from mobility equipment and vehicles
Reliable vehicle for transportation of individuals with developmental and/or intellectual disabilities
Fluent in English
Fluent in Spanish (Preferred)
Ability to effectively serve in the role of an essential service worker during state/local emergencies and pandemics
Willingness to travel to see clients
Tasks:
Make effective schedules to serve consumer(s) to fulfill goals and objectives in the Individual Service Plan (ISP).
Manage completion of activities in a timely manner to meet short and long-term goals of the consumer(s) and their dependents.
Attend meetings with consumer(s) and case manager(s).
Maintain consumer(s) confidentiality and responsibilities of a Mandated Reporter and The Health Insurance Portability and Accountability Act (HIPAA).
Keep current and accurate records of service signed by the consumer.
Complete timesheets at the end of each service period.
Complete the mileage reimbursement log at the beginning and end of travel for all direct services.
Maintain and protect Family Works equipment.
Maintain a professional relationship with Regional Center staff regarding consumer(s) matters.
Review consumer(s) and overall case planning, utilizing agency staff and other resources.
Encourage and guide consumer(s) in identifying and achieving personal goals, and when applicable, include relevant involved others (i.e. parents or children).
Develop and maintain consumer(s) notes and progress reports.
Using various approaches, engage with the consumer(s) according to the individual service plan.
Encourage the consumer(s) and family to fully participate in coordinating and utilizing the services/resources available.
Transport consumer(s).
Perform tasks that relate to administrative responsibilities and other duties as assigned.
