Summary of Role:
Every Day Connect (EDC) Service Coordinator - Mobile Van Services
As an integral part of our services team, the EDC Service Coordinator plays a pivotal role in driving our mobile van services to offsite community locations. This position is not just about driving; it's about making a difference in the lives of those we serve.
Key Responsibilities:
Driving and Logistics: As the primary driver for our mobile van services, you will ensure the safe and timely transportation of our team to various offsite locations. You will also coordinate the logistics for daily services and weekly appointments, ensuring everything runs smoothly and efficiently.
Service Coordination: You are responsible for coordinating all Every Day Connect (EDC) programs, including Drop-In SCs, Off-Site SCs, and Core Senses (Vision, Hearing, and Dental). You will also provide primary support within at least one core senses program.
Participant Support: Working directly with participants, you will link them to our in-house programs and community resources through our Drop-In Services. You will also refer participants to various homeless resources in the community, including city services such as the Department of Homelessness and Supportive Housing (HSH) Coordinated Entry System, HSH Homeless Outreach Team (SF HOT), and 311.
Community Partnerships: You will manage relationships with community partners, fostering collaboration and mutual support.
Program Evaluation: You will evaluate the impact of our program, helping us continuously improve and adapt to the needs of those we serve.
