We are constantly growing and now seeking anIntake Coordinatorto join our Hospice Intake team in the San Luis Obispo office. This position will be cross trained to complete various tasks throughout the department and responsibilities will vary day-to-day. Responsibilities include coordinate and schedule all team member care/visits, answer multiple phone lines for a variety of disciplines with accuracy and efficiency, answer questions and assist patients to ensure patient care is planned, coordinated and delivered appropriatelywith the ability to stay on task and re-prioritize workload as needed. Work Schedule: Full-time hours, Monday – Friday 8:00-5:00 (SLO Office) with weekend rotation.
