POSITION TITLE: Residential Case Manager

PROGRAM/LOCATION: COMMUNITY RESIDENTIAL PROGRAM (CRP) - SAN LUIS OBISPO

POSITION SUMMARY:

This position has a client caseload split between Community Housing and Bishop Street Studios. Case Managers for the Community Residential Program are expected to supervise the general health, safety, and social rehabilitation of clients. These residents will all be adults who receive services from San Luis Obispo Behavioral Health. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed. The overall goal of the program is to help adults with serious mental illness succeed in the community with sufficient structure and support. The Case Manager will be responsible for crisis care, individual rehabilitation activities, day-to-day skills building, transportation to appointments, and all other aspects required to help the clients maintain stability. The Case Manager will work Tuesday through Saturday providing a full range of services to the residents of CRP using a trauma informed approach. The Case Manager will work at the CRP SLO office Tuesday through Thursday and the Bishop SLO Office Friday and Saturday.

HOURS/ SALARY:

Full-time, non-exempt position. 40 hours per week. Anticipated hiring range is $24.50-$25.50. (Full Salary Schedule $24.50-$32.00 per hour).

Requirements

Minimum Requirements:

1. BA/BS AND 6 months of full-time equivalent experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time equivalent experience with target population, OR 4 years of full-time equivalent experience with target population.

2. Must have a strong commitment to the right, ability and choice of each person to live in a safe environment; develop a meaningful role in the community; and have access to competent and continuous supports and services.

3. Must have a reliable automobile and utilize own vehicle at work with company mileage reimbursement when a company vehicle is not available.

4. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.

5. Well defined organizational and time management skills.

6. Ability to work independently and flexibly.

7. Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure.

17. Conditions of employment include:

Must be at least 18 years of age; possess a valid CA Driver’s License, at least 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance.

Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting.

Must be able to successfully complete First Aid and CPR training
