We have two congregate facilities that are 1.2 miles apart, with a maximum of six beds per house.
Provide overall direction and supervision of nursing and caregiving staff, including RNs, LVNs, CNAs, and direct support staff.
Currently licensed by the California State Board of Registered Nursing - BRN
Ensure comprehensive assessments and individualized care plans are completed, implemented, and regularly updated for all residents.
Monitor residents’ health status, coordinate medical care, and respond to changes in condition promptly.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Ensure infection control and disease prevention protocols are implemented and monitored.
Participate in interdisciplinary care conferences and support family communication regarding resident care.
Ensure proper reporting and investigation of incidents, accidents, and grievances.
Conduct routine audits of medical records, medication administration, and resident care plans.
Prepare for and participate in state inspections, surveys, and audits.
Ability to manage multiple priorities in a fast-paced environment.
Must be able to communicate and work professionally and cooperatively with others.
Successfully pass all pre and post-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
Must visit the facilities a minimum of three times per week and submit complete weekly notes without exception
