Potential Shifts Available: 7am-3pm, 3pm-11pm and 11pm-7am, weekdays and weekends
A consistent weekly availability of 2–3 shifts is required

Responsibilities
Provide direct patient care in accordance with individualized care plans.
Administer medications and treatments as prescribed by the physician.
Monitor patients' conditions and report any changes to the healthcare team.
Assist with daily living activities, including personal hygiene and mobility support.
Educate patients and their families on health management and wellness strategies.
Maintain accurate electronic documentation of patient progress and interventions.
Participate in ongoing training and professional development opportunities.
Qualifications
Current LVN license in the state of California.
Experience in Nursing
Experience with Special Needs, Autism, Ostomy Bags and Insulin
Ability to provide critical care for patients with complex needs.
