SOCIAL WORK CARE COORDINATOR for an Industry Leader in Healthcare

DUTIES Include
Performs new client intakes with seniors, or their loved ones, by phone.
Selects appropriate cases to pursue.
Monitors caregiver performance for follow-up on service delivery. Determines satisfaction of services.
Participates in case conferences.
Performs case management.
Provides scheduling, when needed.

REQUIREMENTS
A very caring, outgoing, upbeat, people-person.
Excellent multi-tasking and organizational skills for this fast-paced environment.
Coordination, scheduling, staffing, human resources, office supervision, logistics, administrative or resource allocation experience.
Excellent multi tasker
A strong work ethic
Superior oral and written communication skills.
Demonstrated good judgment and excellent problem solving ability.
One of the following credentials/licenses: MSW, BSW, M.A. Counseling, M.A. Psychology, B.A. Psychology, LVN, or RN (or similar degree/credential)
Medical industry experience of some kind is required.
Please live a relatively easy commute from Newbury Park. We are 1 minute off of the 101 freeway, at the Wendy Drive exit. We are 20 minutes from the Sycamore Drive exit in Simi Valley, off of the 118 freeway. We are 19 minutes from the Valley Circle Blvd exit in West Hills, off of the 101 freeway.
