STATEMENT OF PURPOSE (THE POSITION):

Under the direction of the Program Director, the Social Worker provides direct and indirect social work services to participants and helps participants address their social, emotional and psychological needs along with providing and developing and /or assisting in the access of services to meet those needs. The Social Worker is responsible for fostering an environment and policies that provide our participants with a supportive, nurturing, family-centered environment that promotes wellness, independence, self-esteem and recognizes and celebrates the value of the individual. This environment shall provide participants with the highest practical level of physical, mental, and psycho-social well-being and quality of life.

PRIMARY DUTIES AND RESPONSIBILITIES:

Conducts initial and continuing psycho-social assessments for the development of individual treatment plans including goals, objectives and services designed to meet the needs of the person.

Provide full case management focusing on the short- and long-term goals with participation of the participant’s family and other appropriate individuals

Develop and lead problem-oriented discussion groups among participants

Provide referral and follow up for referred persons for services needed beyond participation in adult day health care services

Assist with coordination of participant schedules of attendance and access to program services

Act as an advocate for participant concerns and community liaison.

Research and identify community resource available to our population. Maintain and manage community resource manuals up to date

Provide participant and family counseling as needed.

Complete all intake processes and discharge planning.

Participate in all multi-disciplinary meetings.

Facilitates communication among program, home and community providers.

Works in close cooperation with staff and participants as a member of the Multidisciplinary Team.

Records initial and final evaluations, makes daily progress notes, and schedules participants for further treatment.

Supervises the work of Social Worker Assistants.

Additional tasks as provided

MINIMUM EDUCATIONAL REQUIREMENTS:

Master’s Degree in Social Work from an accredited school of social work

OR

Master’s Degree in Psychology, Masters in Gerontology, Masters in Counseling from an accredited school of social work and has 1 year of experience providing social services in one or more of the fields of aging, health, or long-term care services.

OR

Bachelor’s Degree in Social Work from an accredited school and has 2 years’ experience providing social services in one or more of the fields of aging, health, or long-term care services.

OR

The person is licensed by the California Board of Behavioral Sciences.

SKILLS DESIRED

Bilingual a plus.

Experience in working with varying medical conditions such as physical impairments, developmental disabilities and frail elderly populations a plus.

Good communication, people, and organizational, and leadership skills.

Knowledgeable of Title 22 California Code of Regulations

Experience in conducting case management services and familiarity with available community resources for the population served

Job Type: Full-time

Schedule:

8-hour shift (Monday – Friday). No Weekends!

Work Location: In person
