Synergy specializes in treating adolescents ages 12-18 who struggle with co-occurring substance use and mental health issues. Synergy's clinical team specializes in delivering evidenced-based therapy models in an outpatient setting with the majority being in-home/community models.The Care Navigator position will work to support our clients and help navigate their care, with specialized attention to clients with opioid use disorders as this population is at very high risk and has elevated needs. This position will conduct a social determinants of health screen and identify needed resources, which may include primary care, infectious disease services, education programming, job training programs, food security, social services, and sexual health and family planning services. This position will help with care coordination, referral tracking and continuity of services and care. This will include working to engage our clients in their care within the agency (increasing attendance at psychiatric appointments, therapy appointments) as well as helping coordinate needed care in the community. The Care Navigator will help clients and their families overcome barriers by providing knowledge and assistance to help "navigate" systems in order to access quality care and services as well as provide support to help retain them in treatment.Synergy's main offices are located on the Ft. Logan campus in Denver; however, Synergy has a satellite office at the Family Resource Pavilion (FRP) located at 9700 E. Easter Lane in Centennial, this position is a hybrid position that will work a combination of remote work and office/on-site work at our offices on Ft Logan and potentially FRP.Key Responsibilities:

Conduct Social Determinants of Health Screen with clients and identify needed resources.

Communicate and follow up (in-person, by phone, text, or email) with clients and families regarding coordination of care and referrals to community resources.

Provide appointment reminders, follow up and help coordinate their psychiatric care within our team.

Assist with care coordination and referrals for psychiatric and other needed services and support in the community during treatment and when clients are preparing for discharge.

Provide transportation for psychiatric appointments to clients/families who have transportation barriers.

Collaborate with Synergy's treatment team to assess client's needs and customize appropriate internal and external resource needs.

Research community resources in order to coordinate external needed services (medical, infectious disease services, education programming, pro-social, job resources, Medicaid applications/renewals, gang reduction resources, community treatment providers for aftercare referrals, sexual health and family planning resources, housing/financial resources, food security & social services etc.).

Help establish and update a resource list that can be used by the clinic to help support families.

Attend community meetings/provider resource fairs, attend trainings, committees, etc., as directed by supervisor.

Maintains accurate, thorough, and timely records of all activities, including documenting all case-management and care coordination for clients.

Assist in completing admission paperwork with clients, as needed.

High school diploma or General Education Diploma (GED).

One (1) year experience in case management or care coordination.

Applicants must meet minimum qualifications at the time of hire.Preferred Qualifications:

Bachelor's degree in human services, psychology, counseling, social work, or a related field.

Bilingual English/Spanish.

Experience working with adolescents.

Experience working in a behavioral health field.
