Job Summary:Coordinates management of skilled nursing facility care, through a collaborative relationship with physicians, facility staff and other members of the health care team. Is accountable for maintaining defined goals through case management, utilization management and discharge planning while assuring appropriate services are delivered. Position requires travel to assigned facilities and flexibility to cover multiple sites if assigned.Essential Responsibilities:

Responsible for the daily oversight and coordination of care for KPCO members in a Skilled Nursing Facilities

Develops and participates in ensuring discharge plans are being actively implemented including scheduling and facilitating family conferences, identifying, and coordinating outside community resources in partnership with facility staff.

Develops and implements systems and models of care to ensure high quality, cost-effective outcomes for programs within areas of responsibility. Defines and assesses problems; develops and implements solutions for providers, staff, and contract service providers for aggregate populations.

Provides professional consultation for complex situations to internal providers, staff, members and community service agencies.

Takes lead in fostering partnerships and relationships with facilities and acts as primary liaison

Collaborates with Utilization Management in the coordination of care for LTACH and Acute rehab as appropriate

Develops and implements programs for changing organization and member needs.

Basic Qualifications:

Experience

Minimum two (2) years of experience in coordinating/planning or delivering appropriate care in public/home health/long-term care, skilled nursing facility, rehabilitative/acute care, or hospital discharge planning.

Education

Bachelors degree in a health profession, social work or closely related field required.

License, Certification, Registration

National Provider Identifier required at hire

Additional Requirements:

Working knowledge of hospital discharge planning, community home health resources, skilled nursing, hospice facilities, and Medicare/Medicaid guidelines.

Facilitation, conflict resolution and negotiating skills are required.

Appropriate licensure or certification.

Examples of preferred health profession credentials are RN, LCSW, Physical Therapist, Occupational Therapist, or similar health specialty.

Preferred Qualifications:

Masters degree preferred.
