This position is contingent on contract award.This position will support the US Department of Veterans Affairs Critical Staffing Program.  The person in this position will represent International SOS and provide services outlined below: International SOS is looking for an individual who is an experienced Referral Coordination Registered Nurse to manage and coordinate referrals for patients within a healthcare setting, ensuring timely access to specialized care, tracking referral progress, and collaborating with healthcare providers to enhance patient outcomes and continuity of care. Key ResponsibilitiesProvide direct and indirect nursing care, by contributing to the patient’s assessment, determining priorities, identifying nursing measures and therapeutic objectives and evaluating outcomes.  Apply independent judgment and nursing actions, flexibility, organizational skills, and the ability to utilize an interdisciplinary approach in the provision of nursing care. Triage specialty consults for clinical appropriateness and completeness within the National RCI Standards for coordinated care.  Communicate with veterans on their health care options via phone calls to determine location for care at VA facility or through the community eligible providers allowing the veterans to make informed decisions on where to receive care if they are eligible.  Provide clinical knowledge and act as a clinical resource to multidisciplinary staff within our VISN19 and outside the VISN.  Collaborate with CACs to rebuild and modify consult templates.  Serve as a liaison to both internal staff and community care staff during the consult process.  Serves as liaison to the ordering provider and specialty care services.  Serves as delegation of authority by building trusting relationships with the specialty providers in the clinics  Vet RCT Specialty Triage Tools with the VA specialty providers.  This list is non-exhaustive, and the role holder may be required to undertake additional duties that are not specifically listed above.
