We're looking for an experienced MDS Coordinator to join our Skilled Nursing Neighborhoods and help lead the way our residents, care partners, and interdisciplinary teams work together every day.This is an on-site role. Our MDS Coordinators are key leaders in our community, bridging clinical accuracy with compassionate, person-centered care.Position SummaryThe MDS Coordinator partners across multiple departments to coordinate the development, completion, and ongoing updates of resident assessments and care plans, in accordance with state requirements and the policies and goals of Christian Living Communities. This role monitors and guides the completion of PPS, OBRA, and CAA assessments, steps into direct nursing care when needed, and supports leadership during surveys and inspections by authorized government agencies.Essential DutiesOversees and evaluates the gathering of information for assessments and care plans; schedules and facilitates care plan conferencesMonitors and guides the completion of PPS, OBRA assessments, and CAAsMaintains and periodically updates written policies and procedures governing the development, use, and implementation of the resident assessment (MDS) and care planEnsures timely and accurate transmission of completed MDS to appropriate parties on a weekly basis; maintains 15 months of completed MDS on all residents for surveyorsPerforms assigned duties as part of an interdisciplinary teamPerforms the role of a nurse when needed and as assignedCompletes administrative duties such as medical forms, reports, evaluations, and studies as necessaryDevelops and maintains an ongoing quality assurance program for resident assessments and care plansParticipates in on-site surveys conducted by authorized government agenciesOther duties as assignedQualificationsActive RN or LPN license in good standingPrior MDS coordination experience strongly preferredWorking knowledge of PPS, OBRA, and CAA assessment processesStrong organizational skills and attention to detailAbility to collaborate effectively across nursing, therapy, social services, and other departmentsExcellent communication skills, both written and verbalWhy Join UsAt The Suites of Someren Glen, you'll be part of a thriving, respectful, and collaborative team where your clinical expertise directly shapes the quality of life for our residents.
