The RN Case Manager reports directly to a Clinical Supervisor and is responsible for care coordination of clinical staff, patients and their families/caregivers, and physicians or other care providers. Responsible for admissions, recertification assessments, transfers/resumptions of care, and discharge assessments, including supporting documentation. Duties and Responsibilities   Ensures direct supervision of CNAs, LPNs, and RNs in the field. This involves training and re-education of staff on clinical skills/procedures when necessary.Develops an individualized plan of care for each client.Provides support, guidance, and education to patients and their families/caregivers to assist patients in their progress toward their goals.Completes all clinical documentation in a timely and accurate manner, and ensures all documentation meets the requirements of agency policies and state regulations.Complies with and participates in continuing quality improvement by reporting incidents, injuries, and infections.
