Our established R&A healthcare facility is in search of an experienced and compassionate Assistant Care Coordinator who understands the importance of meeting the needs of clients while helping them maintain a high quality of life. We are looking for committed individuals who have impeccable communication skills, and can problem solve on the spot. Our employees have a reputation for being cheerful and receptive as they administer non-skilled care to our Special need’s clients. We are seeking someone who would be a great fit with our team, who is kind, caring and compassionate.

Job Duties Include But Not Limited To:

- Review each assigned case care plan with the Care Coordinator to assure clarity on the required duties/tasks, schedules and special needs of each patient.

- Monitors delivery of care by making monthly home visits to supervise assigned Care Providers work and document patient status.

- Reports personnel vacancies or performance issues with the Care Coordinator by maintaining ongoing communication regarding variances from patient care plans.

- Protects self, co-workers, and patients by following policies and procedures to prevent the spread of bloodborne and/or airborne diseases.

- Respects patients by recognizing their rights; maintaining confidentiality.

- Maintains quality service by establishing and enforcing organization standards.

- Complies with R&A’s philosophy, goals, and standards of care; requiring adherence to the Colorado Department of Health licensing rules and regulations and other governing regulations.

- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies.

Minimum Qualifications:

Be at least 18 years of age

Have appropriate experience or training in the home care industry or closely related personal care services.

Have completed training in the provision of personal care services or be willing to complete training once hired and obtain a successful score for passing.

Ability to work with and supervise others as an effective team builder and team player.

Excellent analytical and problem solving skills.

Knowledge of principles and procedures of performing supervisory duties for homecare.

Ability to communicate effectively, verbally and in writing.

Ability to establish and maintain effective working relationships with clients and families, staff

members, and the general public.

Special Requirements:

Valid and current Colorado Picture ID and Social Security.

Clear criminal background history.

Access to reliable public or personal transportation.

The services we provide are non-skilled for our in-home clients with special needs range from children to seniors who have been approved for LTC through medicaid. Typical duties include companionship, meal preparation, light housekeeping, shopping, personal cares, respite, transportation, etc.

We will train a right person. However, experience of working with Adults with the Mental Deficits is a plus.
