Job Summary:
The Home Care Registered Nurse is responsible for providing skilled nursing care to clients in their homes, ensuring high standards of care in line with individualized care plans. This role includes assessing client needs, performing nursing tasks during home visits, collaborating with multidisciplinary teams, and developing, reviewing, and signing care plans in accordance with clinical and regulatory requirements.

Key Responsibilities:
Clinical Duties:
Conduct comprehensive nursing assessments during home visits.
Provide direct patient care including wound care, medication administration, catheter care, vital signs monitoring, and chronic disease management.
Monitor and evaluate patients' responses to treatment and adjust care as necessary.
Educate patients and families on disease processes, self-care techniques, and use of medical equipment.
Care Plan Management:
Develop and implement individualized care plans based on nursing assessments and input from other healthcare professionals.
Regularly review, update, and sign care plans to reflect changes in the patient’s condition or needs.
Ensure care plans meet all clinical standards and comply with relevant legal and regulatory requirements.
Coordination and Communication:
Liaise with GPs, hospitals, social workers, and other healthcare providers to coordinate care.
Maintain accurate and up-to-date clinical documentation and records.
Communicate effectively with clients, families, and the care team.
Compliance and Professional Standards:
Adhere to policies, procedures, and professional codes of practice.
Maintain up-to-date CPR certification and nursing registration.
Participate in quality assurance, audits, and continuous improvement initiatives.
Qualifications & Experience:
Registered Nurse with active license
Minimum 1 year of nursing experience, preferably in community or home care settings.
Strong clinical skills and experience in care plan documentation.
Valid driver’s license and access to reliable transportation for home visits.
