Holds a valid Colorado State Nursing License as a Registered Nurse.
Possesses a minimum of three years of clinical nursing experience, specializing in end-of-life care.
Prior experience in clinical leadership or supervisory roles preferred.
Prior experience with CHAP, ACHC or JCAHO preferred.
Capable of self-direction and autonomous work.
Skilled in clinical coordination and communication.
CLINICAL JOB FUNCTIONS/RESPONSIBILITIES
Conducts rigorous clinical assessments of cases, evaluating diagnosis, prognosis, pharmacotherapy, and overall trajectory.
Offers structured guidance to clinicians to uphold quality standards.
Partners with clinical staff for care goal setting, prioritization, and comprehensive planning.
Oversees or assists in clinical staff performance evaluations.
Chairs or partakes in clinical case conferences, promoting interdisciplinary collaboration.
Spearheads the development and refinement of clinical protocols, guidelines, and organizational policies.
Strictly adheres to professional clinical standards and ethical guidelines.
Advocates for a customer-centric approach within the clinical team.
Engages in clinical quality assurance and performance improvement, providing leadership and expertise.
Commits to ongoing professional development through clinical workshops and participation in clinical networks or societies.
Undertakes additional tasks as assigned by the VP of Clinical.
