Direct Support Professional

Job Summary:

Under the supervision of the Program Director, DSP’s are responsible for helping people with intellectual/developmental disabilities connect with their community through activities such as community events, recreational activities and volunteer opportunities, also assist individuals in learning and maintaining life skills to improve levels of independence. We provide support designed to improve levels of independence for individuals with developmental disabilities in our Program Site and in the Community at large.

DSP’s will ensure the people have opportunity to experience activities that are enriching, meaningful and not day wasting. DSP’s will ensure that venues accommodate and are safe for people we support. DSP’s will ensure the happiness and well-being of people receiving services through Thrive Community Options. DSP will implement programs and activities designed to meet the individualized needs of our participants; orient new individuals to the program and coordinate daily schedules and activities; Assess and document member information; transporting and monitoring members to and from program sites.

Essential Job functions are but not limited to:

Help identify community resources and activities pertaining to the interest and skill level of people.

Promote Community Integration by ensuring the same activities and venues are utilized as those used by the general public.

Foster connections between people we support and the community at large to improve social skills.

Implement programs and activities designed to meet the individualized needs of Participants,

Coordinate daily schedules and activities to maximize individual participation,

Coordinate and implement programs outlined for assigned individuals

Supervision of all individuals throughout the day to ensure their health and safety,

Provide supports to maintain or improve independence.

Provides care related to feeding, personal care, behavioral supports, medication administration, etc.

Maintain a safe, sanitary, and organized environment for individuals

Maintain an environment that is stimulating and conducive to promoting Independence and individual success

Evaluate new individuals and implement interim care plans until Service plan is completed report findings to supervisor.

Orient new individuals to the program

Executes and Maintains comprehensive documentation for people in services including but is not limited to; ISSP’s and BISSP’s tracking's, progress notes, incident reports, daily activities, attendance, and other necessary agency documentation;

Communicate with Program management frequently to share information and resolve issues;

Work with other staff to share ideas, seek assistance;

Changing and ensuring cleanliness of participants that use adult undergarments;

Performing G-tube feedings (training provided) and assisting others with feeding;

Ensure proper storage and care of equipment and supplies;

Act as an advocate for assigned individuals by ensuring communication regarding the care of people with the Thrive staff, Host Home Providers, and or guardians and families.

Provide transportation and monitoring of individuals;

Operate vehicle within the rules and regulations outlined by company policy and Department of Transportation guidelines; conduct vehicle inspections daily, complete necessary documents as vehicle inspection form, mileage and air mileage logs, other transportation related agency documentations.

Promote person centered approach and support by empowering each person’s needs, desires and choices.

Prevent and report Abuse, Neglect, Mistreatment, Exploitation promptly;

Performs other duties assigned.
