Women’s Wellness Connection Patient NavigatorAs a grantee of Colorado Department of Public Health and Environment (CDPHE), Salud is able to offer the Women’s Wellness Connection (WWC) Program to eligible patients for cancer prevention and early detection. The WWC Patient Navigator functions as a member of the patient care team, performing patient navigation for both insured and uninsured women through the Health Navigation and Clinical Services component of the WWC Program.
