CDHSCareers About Grand Junction Regional Center This work unit provides comprehensive services and support to individuals with intellectual and developmental disabilities. The unit’s focus is on providing services with the support of an active learning approach while also focused on creating and promoting a safe and healthy environment for individuals supported. This work unit provides 24-hour treatment services that are in compliance with State and Federal regulation to ensure a home-like environment for people with developmental disabilities or who have a dual diagnosis, ensuring personal outcomes and achieving a therapeutic milieu for up to 12 persons. Description of JobAbout the Position:This position ensures all residential programming is implemented in the home. This includes providing supervision, modeling, and coaching to direct support professionals. This position spends the majority of the time in the home ensuring the milieu is operating to efficiency and works closely with the SSC/QIDP to ensure the implementation of services is occurring within the correct guidelines. Duties in this position include:Supervision:
supervise up to 15 health care technicians assigned to residential sites through direct and delegated authority
as a supervisor and role model, monitors and provides hands on care and/or instruction in daily living activities for persons receiving services to ensure safety and personal outcomes are met as determined by their Individual Plans
act as liaison between direct care staff, managers, and therapies
may be required to work other locations and/or shifts as needed to address agency resource needs
complete Performance Management Plan with steps for professional improvements
Program Implementation:
monitor and implement individualized supports and services for people. These programs include behavior, social, recreation, work and specialized therapies in residential, day program areas and community settings
respond to behavioral challenges as needed with implementation of specific Behavior intervention programs (including Safety Control or Emergency Control)
annually certified for the skilled implementation of Safety Care, Interventions
are to be from the least invasive and restrictive and only progressing should safety to a person or peers be imminent
assists to identify, in-service and evaluate staff on training needs for achieving outcomes
follow defined documentation guidelines for daily use of electronic health record for documentation
Compliance with Agency Requirements:
plans for, audits, and allocates resources to meet annual and daily needs of residential site (this includes, but not limited to budget, staff scheduling, medical needs, transportation and needed supplies)
identifies, evaluates, and ensures compliance to Agency Policy and Procedure requirements
Environment:
monitor the physical condition of the living site by addressing safety concerns, making purchases, decorating living area and performing housekeeping duties
assist individuals with decorating their personal space.
as assigned completes the quarterly Environmental Checks
Best Possible Health:
ensures the best possible health by administering and supervising the implementation of treatments, medications, therapy supports and dietary orders as prescribed
responds to emergency situations and assists medical staff with medical support and services
ensures that meals are prepared to prescribed diet consistencies as specified in the
dietary planning menu. Variances to orders must be documented and as needed have responses as directed by Health Services staff.Administrative/Training:
collects, completes, monitors, and delegates administrative forms, reports and records in order to achieve ongoing licensure and certification.
completes these duties using prescribed electronic and hard copy procedures utilizing computers and other supporting tools as needed to complete the duties
participates in agency work groups, committees, and attends assigned meetings and in-services as scheduled.
annual recertification is required for “CORE” skills including CPR, suctioning, transfers and lift use, wheelchair securement, Safety Care.
additional mandatory training is presented through Continuing Education on an annual basis
QAPI (Quality Assurance Performance Improvement):
participates in the QAPI process at the home and day program as directed by your leadership team
utilizes the most current version of forms, policies, procedures, addendums, and form instructions
documents changes in individuals’ condition and significant events identified in team notes, ensures medical conditions are identified and followed up occurs, follows the individual support plans as well as the individual behavior support plans
documents all information objectively and accurately
provides documentation that is descriptive, objective and person centered focused
Other Duties as Assigned:
may be required to work other locations and/or shifts as needed to address agency
needs
may be required to drive state vehicles with the person who live at GJRC.
must travel between group homes to provide care and training, and to transport individuals as needed.
when not transporting individuals, this person may drive a State vehicle or choose to drive their own
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal RightsMinimum Qualifications:A high school diploma or GEDANDThree (3) years of experience providing direct care services to developmentally disabled individualsSubstitutions:Additional appropriate education will substitute for the required experience on a year-for-year basis.Preferred Qualifications:
demonstrated computer skills and experience
one (1) year of supervisory experience
demonstrated experience managing living site budgets/money management
Conditions of Employment:
Full background check required. CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers).
Must possess a valid, U.S. driver's license
GJRC is a 24/7 facility, must be available to work any shift, any day and/or any area, to meet the a agency needs
Travel - Must travel between group homes to provide care and training, and to transport individuals as needed. Position is required to use a State vehicle to transport residents. When not transporting individuals, this person may drive a State vehicle or
choose to drive their own.
Other: TB test required; Requires Influenza and COVID vaccine as required by state and facility
Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
