All while understanding that together we are even better.Schedule: Full TimeShift : DayThe role you’ll contribute:The Transfer Coordinator will coordinate all processes, support, communication and documentation involved in transferring patients from one facility to another AdventHealth facility, as well as support physician consults. Transfer Coordinators are also required to recognize and uphold all regulatory requirements, while simultaneously updating key stakeholders of timelines.The value you’ll bring to the team:
Coordinates requests for transfer of patients into the Centura Health system, or requests for medical consultation, including connecting the referring MD with the accepting/consulting MD.
Coordinates with receiving facility bed availability for the patient.
Professionally and accurately represent all AdventHealth facilities. Ability to develop and maintain referral relationships with outlying facilities through consistent exceptional customer service.
Facilitate all communications between referring and receiving physicians and other hospital staff, while relaying information accurately.
Accurately document and enter all required information into the EPIC database as it occurs, for creation of real time data collection.
Demonstrate exceptional customer service and support with every call.
Supports referral patterns in collaboration with Program Director and Medical Director. May be asked to suggest a receiving site based on clinical expertise of a patient condition.
Has a strong understanding of the geography of our acute care facilities. Understands the transportation requirements for patients referred from our rural affiliates and sites that frequently refer patients to our facilities.
Provides follow up to referring physicians as outlined in procedures, on patients that were transferred in to the Centura Health system through the Transfer Center service.
QualificationsThe expertise and experiences you’ll need to succeed:Minimum qualifications :
High School Diploma
BLS certification
1-2 years direct clinical experience in an Emergency Department/Critical Care Unit
1-2 years of full-time direct administrative healthcare experience (Nursing Unit Secretary or Patient Access Representative)
1-2 years of Transfer Center experience
Preferred qualifications:
Bacehlor's degree preferred
