Responsibilities

Assessing the psychosocial status of patients and families/caregivers related to the patient’s illness and the environment and communicates findings to the registered nurse and other members of the interdisciplinary group

Carrying out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plans interventions based on evaluation findings

Counseling patients and family/caregivers as needed in relationship to stress and other identified coping difficulties, providing crisis intervention when necessary

Assessing for any special needs related to the culture of the patient and family

Maintaining clinical records on all patients referred to social work

Participating in the development of the individualized plan of care involving the patient and family, and attending regularly scheduled interdisciplinary group meetings

Assisting physicians and other team members in understanding significant social and emotional factors related to health problems and death/dying issues

Assisting family and patient in planning for funeral arrangements, financial, legal and healthcare decision responsibilities

Ensuring compliance with all state and federal legal and regulatory requirements

Additional responsibilities as assigned

Qualifications

Graduate of a Master’s program in social work accredited by the Council on Social Work Education

Current licensure according to Medicare guidelines to practice in state of operation

1+ year experience in healthcare social work

Experience in home health or hospice preferred

Excellent verbal and written communication and organization skills

Commitment to clinical and documentation excellence

Salary: $55000 - $85000 / year dependent on experience Schedule: Full-Time, Mon-Fri, 8:30am-5pm + on call rotation We are looking for a minimum of LSW or LCSW in the state of Colorado Responsibilities Social Worker I Responsibilities Assist the core members of the hospice team in understanding significant social, spiritual, and emotional factors related to the patient\'s health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient. Participate as a member of the interdisciplinary team and in the development and review of the plan of care for assigned patients. Assess the social, spiritual, and emotional needs/factors in order to estimate the patient\'s and involved caregiver\'s capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness. Help the patient, family, and involved caregiver to understand, accept and follow medical recommendations and provide services planned to assist in achieving the optimum social, spiritual, and health adjustment within their capacity and level of health literacy. Prepare the patient to deal with the changes and the family to support the patient including education on Advanced Directives. Utilize all available resources, such as family, hospice, and community agencies, to assist the patient and family to live better within the limitations of the illness.
