Job DescriptionEMS Unlimited is preparring to launch a Community Paramedic / Mobile Integrated Health (CP/MIH) program in Garfield County, we hope to start seeing patients in early 2027. We are seeking a new team member to join soon and to be a part of the building process before it goes live. This is a full-time Community Paramedic role. You will work out of a dedicated CP vehicle, see enrolled patients in their homes, and work alongside their physicians to close the gaps that send people back to the emergency department. You will practice under Colorado's Community Paramedic endorsement, CP protocols, and real-time telehealth access to a supervising physician.Who you'll seePatients recently discharged who are at high risk of readmission (CHF, COPD, pneumonia, diabetes, post-surgical)Frequent emergency department users and patients without a stable primary care connectionPatients living with chronic disease who need help managing it at homeOlder adults at risk for fallsPatients with behavioral health or substance use needs who need follow-up and connection to servicesWhat you'll doAssessment and clinical careComplete a full in-home history and physical exam, including vital signs, cardiac monitoring, and 12-lead ECG acquisition and interpretationPerform point-of-care testing (i-STAT and glucose) and venipuncture/specimen collection as orderedUse point-of-care ultrasound within protocol and trainingProvide wound assessment and care, IV access, and medication administration under CP protocols and physician ordersRecognize decline early, escalate by telehealth consult, and arrange the right level of care (clinic, urgent care, ED, or EMS transport) when a patient needs morePost-discharge transitional careMake post-discharge home visits, typically within 24–72 hours of dischargeDo medication reconciliation: compare the discharge list to what is actually in the home, identify duplications, missed doses, and adherence barriers, and report discrepancies to the prescriberReinforce discharge instructions and confirm follow-up appointments are made and keptChronic disease management and educationTeach self-management for CHF (daily weights, fluid and sodium limits, symptom red flags), COPD (inhaler technique, action plans), diabetes (glucose monitoring, hypoglycemia recognition), and hypertensionTrack patient progress against care plan goals set with the patient's physicianSafety, behavioral health, and social needsComplete home safety and fall-risk assessments and recommend fixesScreen for depression, substance use, food insecurity, transportation barriers, and caregiver strainConnect patients to behavioral health, social services, pharmacy, home health, and community resourcesCare coordination and documentationCommunicate findings to primary care providers, case managers, and discharge plannersDocument every encounter and follow up on referrals until closedTrack enrolled patients, visits, and outcomes (ED visits, readmissions, goal progress) that the program reports to the stateHelp build the program: CP protocols, referral workflows, and monthly quality reviewsScheduleMonday through Friday, day shift - specifics of the exact schedule are still be designedTravel throughout Garfield County during the shift, in a company vehiclePay & Benefits$38.51–$45.98 an hour ($72K–$86K).
