They may provide services within one or more Wheeler program(s), and the relative concentration between and within programs may change with the needs of the agency. F ederally Qualified Health Center (FQHC) in Hartford, CT. Eligible for up to $250,000 in loan repayment through National Health Service Corp (NHSC). SALARY Full Time salary of $118,000-$140,000 based on years of experience Up to a $10,000 sign-on bonus available! EDUCATION AND EXPERIENCE/QUALIFICATIONS Master’s degree in nursing is required. Must have one (1) year of post-licensure primary care experience or urgent care or triage experience or three (3) years of other post-licensure experience. Exceptions can be made for students who complete an APRN rotation at Wheeler, as well as three months of Wheeler’s accelerated residency program. Credential(s) Required: Licensed Registered Nurse and Licensed Advanced Practice Registered Nurse or Physician Assistant in the State of Connecticut. Board Certified as an Advanced Practice Registered Nurse or Physician Assistant as appropriate for the population served. Active DEA controlled substance registration and an active Connecticut controlled substance registration. Ability to acquire and maintain certification in Basic Life Support for Healthcare Professionals no later than 90 days from date of hire. ESSENTIAL DUTIES AND RESPONSIBILITIES
Conducts medical evaluations and formulates accurate diagnoses in accordance with the International Classification of Diseases. Manages prescribed medication, performs physical health examinations, conducts preventive screening and care, develops and presents clear clinical impressions and recommendations that are specific and attainable. Develops and documents clear clinical formulations and medical treatment plans.
Prescribes therapeutic interventions appropriate to identified problems. Administers drugs and biologicals as required by the Clinic.
Demonstrates knowledge of physical health concerns and effective medical interventions. Triages clinical emergencies and implements safety and consultation protocols. Utilizes evidence based and best medical practice.
Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education.
Provides requisitions for clinical studies, diagnostic tests, and laboratory studies. Evaluates and interprets results to identify and assess patient's clinical problems and health care needs.
Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for consultation with specialists, subspecialist, and allied health professionals as indicated.
Ensures all patients’ medical records are completed timely and in accordance with organization policy and that all charting is comprehensive and legible.
Works with care coordination team in designing, monitoring and implementing care plans. Communicates referrals and collaborates on development of services planning and implementation.
Maintains consistent communication with community providers, local and state agencies, and other allied professionals and services mutually involved in client care.
Represents the Clinic in external and internal committee meetings as assigned.
Communicates in a professional, positive, effective, and courteous manner with client, family, and all internal and external providers.
Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols. Attends and is prepared for scheduled meetings with administrative supervisor.
Attends and is prepared for scheduled clinical meetings and consultation with supervising physician. Seeks additional supervision or consultation as needed and follows through with clinical directives.
Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.
Must be able to pass a respirator fit test at time of hire and on an annual basis.
Providers will treat HIV, Hepatitis C, and provider gender affirming care to Wheeler patients. Support will be provided by the medical director and other provided resources.
Provide services on the mobile unit, as required.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
Regulatory and Corporate Compliance Responsibilities
Understands and adheres to Wheeler Clinic’s compliance standards as they appear in the clinic’s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.
Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc) and specific to the department and position.
Keeps abreast of all pertinent federal, state and clinic regulations, laws, and policies as they presently exist and as they change or are modified.
Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
Performs other related duties as assigned.
