This is affiliated with our Medical Group Practices across the region.Licensed Practical Nurse (LPN) license required.Position PurposeThe Clinically Integrated Network (CIN) Community Health Coach acts as liaison between patients and healthcare and other community services, assisting patients to advocate for healthcare and social needs. Part of a larger population health team who are collectively accountable for coordinating patient care, improving health outcomes and disease management.What you will do

Manage a patient caseload and provides education, screening, brief intervention, and/or facilitation of referrals to providers and community-based organizations and programs

Complete initial assessment for patients identified as rising risk or who have been referred for interventions

Partner with the patient, family, and care team to reduce the impact of the social determinants of health and improve patient's participation in wellness

Serve as local expert in navigating community services available

Collaborate with care team members, including the Primary Care provider (PCP) and their clinical support staff, to understand patient risks and quality gaps in care. This may include visits to aligned primary care offices to meet patients and collaborate with PCP on referrals and support services.

Minimum Qualifications

Current active Licensed Practice Nurse license.

Minimum three years experience in healthcare setting

Community Health Worker Certificate highly desired, or experience with navigating or referring to local community resources a plus.

Bilingual Spanish/English desirable.
