Job Title: Community Response Community Health WorkerJob DescriptionJoin our dedicated team as a Community Response Community Health Worker, where you will perform vital outreach and educational activities in high-need neighborhoods. Your role involves engaging with high-risk individuals, including those who use drugs or engage in sex work, to foster better community relations and improve our service delivery. You will conduct culturally appropriate health promotion, provide crucial harm reduction education, and distribute and collect health-related resources. Additionally, you will connect participants to essential services such as medical care, mental health treatment, and substance use programs, ensuring their access to life-stabilizing resources. Your work will also include preparing and managing program sites, maintaining program supplies, and ensuring the integrity of our outreach efforts. This role requires flexibility to work unconventional hours, including late evenings and weekends.Hard Skills

Associates degree or equivalent experience

Minimum three years of experience in client case management

Valid driver's license

Certification as a Community Health Worker (preferred)

Soft Skills

Strong interpersonal and communication skills

Ability to engage with diverse populations

Mentorship and team collaboration

Adaptability to varied work schedules
