MBI is seeking dedicated contractors who want to work independently to help facilitate change and growth for adults and children that have serious mental illness and disabilities as Community Support Workers/Case Managers.  Community Support Workers spend at least 70% of the time in the field teaching clients the skills required to manage their disorder and succeed in their individual recovery goal.  Community Support Workers support individuals in attaining meaningful, sufficient lives through the growth of the skills necessary to live independently, acquire employment, complete their education/training and have vigorous social lives. Major Duties and ResponsibilitiesAs a Community Support Worker (CSW), you will work independently and respond to the Clinical Manager of your assigned Team. You will be responsible for independently performing a variety of tasks both clinical and administrative in nature.You shall exercise professional, independent judgment to perform the following responsibilities:Maintaining confidentiality of records relating to consumers’ treatment.Collecting information about consumers through interviews, tests, and evaluation of day-to-day functioning within work, family and other routines. Identifying areas needing continued support, resources and treatment in order to assist consumers. Aiding consumers in their own homes/program and in the community to allow them to function more safely and independently. Facilitating physical, recreational, social and educational activities. Providing consumers with a positive role model.Identifying social, economic, recreational and educational services in the community that will meet consumers’ needs. Maintaining liaison with other agencies, professionals, government officials and the community.Recognizing potential crisis situations, analyzing such situations accurately, developing strategies to deal with such situations, and informing Supervisor and Manager when such incidents arise.Recognizing potential emergency situations, analyzing situations accurately, and developing strategies to deal with such situations i.e. dealing with consumer’s aggressive behavior to ensure no harm comes to the consumer, staff and the public.Assisting case management team in identifying potential problems. Providing input to counselors and other professionals with regard to the development of consumer Treatment Plan.Attending worker meetings, supervisory conferences, and other activities.Must do a minimum of 25 billable SIDA (See, Intervene, Document, Approve) hours each week.Attending all company-required trainings.
