BASIC FUNCTIONS: This is a hands-on, client-focused role requiring the incumbent to coordinate with various individuals, CSA teams, and outside agencies to ensure efficiency and a smooth transition throughout the agency intake process, and actively support clients through the continuum of care, including the discharge process. Additionally, this position must be able to complete high-quality intakes and seamless transitions into the CSA/Day Program teams and participate in the prior authorization process for clinical services. The Intake Coordinator will work under general supervision of a licensed mental health professional. PRINCIPAL ACCOUNTABILITIES: Complete enrollment and data entry of clients into the electronic medical record and other programs upon being notified of a client’s engagement. Conduct a psychosocial intake, entering and scanning notes/documents into the electronic medical record as required within same day of service.  Travel within the community to complete intakes with clients when needed. Obtain signed release forms and other required documentation from clients and coordinate appropriate follow-up care with the client’s outpatient providers. Ensure that all transitioning members are able to schedule a Comprehensive Psychiatric appointment and a nursing assessment within 21 days or less upon intake.  Complete required outreach within listed timeframe for consumers who have missed intake appointment. Manage intake schedule effectively to ensure that productivity requirements are met on a daily basis.  Enter any diagnostic information obtained from past service providers into the client’s medical record, attaching any required notes and supporting documentation regarding the diagnosis.  Complete Diagnostic Assessments (DAs) as required for clients' annual requirements. Liaison with clients’ outpatient providers, communicating all relevant information relating to the client.   Create a treatment plan for each transitioning member within day of enrollment.  Complete service notes day of service to include all outreach attempts. Record prescribed medication into clients’ medical record, as required.  Meet with Clinical Supervisor for regular supervision, as required, and update on client’s status post-enrollment.  Make referrals to the Center’s Day Program for clients who need daily structure and rehabilitation. Complete all required clinical and administrative documentation for client discharges. Actively participate in the prior authorization process and submits authorizations through Comagine for services as needed. May perform other duties as assigned.
