Summary:Responsible for the clinical and administrative needs of the neighborhood/s. Coordinates and is accountable for the activities of the nursing neighborhood/s. Implements nursing goals through assessment, planning, organizing, and evaluating nursing interventions. Motivates and directs team members to ensure conformity with nursing philosophy and objectives to attain quality resident care.Essential Duties and Responsibilities:Include the following: Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Maintains compliance with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and community standards. Demonstrates on a consistent basis individual and team behavior that supports the Person-Centered, Woodwell and Woodside philosophies of Presbyterian SeniorCare Network. Consistently upholds the mission, values and behavior standards of Presbyterian SeniorCare Network. Maintains confidentiality. Other duties may be assigned.

Assists in evaluating job performances of neighborhood personnel. Demonstrates supervisory responsibilities as assigned.

Identifies and reports in writing to supervisor any concerns/issues with residents or staff. Counsels LPNs/nurse aides as needed and communicates to supervisor the reasons for counseling.

Ensures documentation on resident records is accurate and communication between shifts occurs to ensure continuity of care. Assures all occurrences are reported to appropriate persons (physician, family, and nursing administration) as per facility policy.

Utilizes current policy, procedure and best practices for prevention of skin breakdown, pressure ulcer treatment, and fall prevention as evidenced by audits of compliance on nursing care units.

Aspires for leadership excellence as exemplified by employee engagement, interdepartmental collaboration, and promotion of a positive work environment.

Provides guidance and feedback to help others to strengthen knowledge/skill to accomplish goals or solve problems.

Performs any or all professional nursing duties including resident assessment and care delivery.

Commits to continually improving his/her skills through participations in education opportunities whether offered on the job, within/without the facility, to increase knowledge of work related issues.

Monitors all clinical orders to assure completion on a timely basis.

Consults the resident's physician in primary resident care, treatment, rehabilitation, etc. as necessary. Notifies the resident's attending physician and next of kin when there is a change in the resident's condition.

Initiates, participates in and monitors the development of each resident's plan of care (preliminary and comprehensive)

Participates in the orientation, training and evaluation of new and existing staff.

Records and monitors documentation on the resident care record to ensure accuracy and regulatory compliance.

Assures all occurrences are reported to appropriate persons (e.g. physician, family, supervisor, and nursing administration) as per facility policy.

Develops and implements in-service programs as requested.

Passes medications and completes treatments in accordance with established policies and procedures.

Follows and promotes infection control policies and procedures. Reports occupational exposures to blood, bodily fluid, infectious materials and hazardous chemicals in accordance with established policies and procedures.

Completes accident/incident reports, including investigation, as required.

Administrates medication and treatments in a timely and efficient manner. Documents accurately in the resident's medical record on medication, treatment records and addresses care plans as needed to insure proper medical/nursing ca e.

Performs all functions within the RN scope of practice.

Participates in Quality Assurance Performance Improvement (QAPI) as requested.

Assists in preparation for regulatory visits.

Exhibits excellent customer service behaviors and interactions at all times.

QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Experience in long terms care preferred.Computer Skills:To perform this job successfully, and individual should have knowledge of Internet software; Spreadsheet software and Word Processing software.Certificates, Licenses, Registrations:Possession of a valid RN license issued by the State of Pennsylvania. Maintains CPR certification per policy.Other Skills and Abilities:Ability to work well with people. Shows enthusiasm and motivation. Ability to be self directed, set priorities, and demonstrate excellent time management skills.
