Full Time, Oahu[Case Manager provides professional counseling and case management services for specific client populations. Position involves evaluating and assessing potential clients needs, linking them to appropriate services and/or assist with placement as needed. Employee must be also able to travel to prospective clients and referral sources sites, conduct regular office/home visits and visits to vendors business locations. The Case Manager must demonstrate an active commitment to Catholic Charities Mission and strive to incorporate the four Core Values in all aspects of daily work.[EDUCATIONAL and WORK REQUIREMENTS: Must have a Bachelors degree in human services, social services or related field plus a minimum of three (3) years experience working with the assigned population.
