As the Home Care Supervisor, you will play a crucial role in planning, coordinating, leading, controlling, and evaluating our home care services. Role Description: The Home Care Supervisor will be responsible for overseeing and managing all aspects of our home care services. This includes coordinating and maintaining effective communication with parent and affiliate departments to ensure seamless operations. The supervisor will also develop policies and guidelines to enhance our administrative systems, working collaboratively with service personnel. They will actively contribute to improving overall efficiency and streamlining processes. Responsibilities: Coordinate and maintain effective communication with parent and affiliate departments to ensure seamless operations. Develop policies and guidelines through collaboration with service personnel, aiming to enhance administrative systems. Offer professional input to improve administrative systems and enhance overall efficiency. Conduct intake meetings and establish personalized care plans for home care clients and their families. Direct and supervise the delivery of client care services by professional and paraprofessional staff. Contribute to marketing efforts and foster positive public relations for the company to expand our reach. Consult and collaborate with home care facilities to address staffing needs and ensure optimal service delivery. Effectively communicate the concept of quality client care to the community, fostering trust and strong relationships. Be readily available during all service hours to address any concerns or emergencies. Perform skilled nursing visits and provide necessary assistance to patients during facility discharges. Act as a client services' administrative liaison by interpreting service needs to the Administrator, ensuring smooth coordination. Collaborate with the Administrator to formulate policies and develop and implement impactful programs. Coordinate with supervisors and staff to evaluate special programs or service programs, identifying areas for improvement. Evaluate the program, services, and field personnel, providing feedback and implementing enhancements. Direct the implementation of service goals and objectives to achieve excellence in care provision. Establish standards to ensure safe and effective services for clients and families, guaranteeing their well-being. Recruit Nurse Aides from various sources such as schools and expos, maintaining a qualified and dedicated team. Maintain open lines of communication with program personnel, fostering a collaborative and supportive work environment. Keep program personnel updated with state-of-the-art clinical information and practices through staff development initiatives, promoting continuous growth. Oversee the implementation and evaluation of client care services, ensuring adherence to regulatory licensure and certification criteria and accreditation standards. Physical therapists will perform physical therapy as needed, incorporating crucial therapeutic interventions. Registered nurses will perform nursing services as needed, ensuring comprehensive and skilled care. Requirements: Hold a valid PT, RN, or LPN license, showcasing professional qualifications in the field.
