JOB DESCRIPTION:less thanbr> The Health Coach will provide individual supportive coaching to clients with chronic mental illness and substance use disorders, focusing on improving their physical health and mental health status. The Health Coach will provide life skills, case management, and structured interventions to assist clients in gaining access to resources and developing skills to improve both their physical and mental health, quality of life, and self-management of symptoms. This will be done by focusing on areas such as health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, and healthcare navigation.
