Collaborate with providers to ensure seamless and timely discharges home, elevating clinical outcomes and patient satisfaction.Responsibilities:Guide patients through post-acute care in the home. Identify those who benefit from home-based services, overcoming health care system barriers. Safeguard their well-being, reducing financial and clinical risks.Advocate for patients during multidisciplinary rounds, fostering holistic care. Qualifications
RN license or Master of Social Work
Home Health experience or prior navigator experience in a post-acute setting such as ALF/SNF/ILF. Combination of Acute and Post-Acute care delivery experience preferred.
Tech-savvy with exceptional time management skills.
