Medication administration, feeding, toileting and skin care, ROM, cognitive stimulation. Additionally the LPN would support the field RNs by making home visits for general assessments, medication set up, wound care, other needed cares, patient/caregiver education, coordination of care with the care team.Position Purpose:Provide 1:1 care for a disabled client in their own home, 2 days a week. Schedule will include in home visits to other patients.
