COMPENSATION RANGE: $30.00 -$40.00 per hour ($62,400.00 - $83,200.00 annually) commensurate on experience and qualifications

SUMMARY OF POSITION RESPONSIBILITIES: The Registered Nurse (RN) will provide comprehensive care to patients in the office based outpatient and assisted living / group home settings. This role combines psychiatric nursing care with the management of elderly or disabled residents, ensuring both mental and physical well-being. The RN will work in interdisciplinary teams, collaborating with healthcare professionals to address mental health conditions while also managing chronic illnesses and daily care needs of residents and patients alike. Registered Nurse is expected to service clients, participate in meetings and conduct trainings as scheduled by the supervisor.
SCHEDULED DUTIES AND RESPONSIBILITIES:
Assessment and Evaluation:
Conduct mental health assessments and monitor symptoms of psychiatric conditions.
Perform physical health evaluations for residents, monitoring chronic conditions such as diabetes, hypertension, and mobility issues.
Care Planning:
Develop, implement, and review individualized care plans tailored to both mental health patients and assisted living residents, ensuring a balance between psychiatric treatment and physical healthcare needs.
Collaborate with healthcare providers, including psychiatrists, physicians, social workers, and caregivers, and family members, to coordinate comprehensive care.
Medication Management:
Administer and monitor psychiatric medications and ensure adherence to treatment plans.
Manage medications for chronic physical health conditions, ensuring correct dosages and schedules.
Patient and Resident Education:
Provide mental health education to patients and their families, including coping strategies, understanding mental illnesses, and medication guidance.
Educate residents and their families about chronic illness management, daily health practices, and general wellness.
Crisis Intervention & Emergency Care:
Respond to psychiatric crises in the mental health setting, offering support and interventions to de-escalate situations.
Handle emergency health situations in assisted living, providing first aid and initiating transfers to medical facilities when necessary.
Wound Care and Basic Health Services:
Provide wound care, manage infections, and ensure the general health and hygiene of residents in assisted living.
Perform routine healthcare services such as monitoring vital signs and managing mobility or physical therapy needs.
Collaboration and Communication:
Work closely with interdisciplinary teams in both settings, participating in care plan meetings and ensuring a holistic approach to patient and resident care.
Communicate regularly with families, keeping them informed of their loved ones' health status and care plans.
Documentation:
Maintain accurate, up-to-date records of both mental and physical health assessments, medication administration, treatments, and any significant changes in patient or resident conditions.
Education and Training: Train caregivers on providing daily care to residents and ensure proper documentation of services.
Emergency Care: Respond to health emergencies, provide first aid, and initiate transfers to hospitals when necessary.
Resident Advocacy: Communicate with family members about the resident's health status and act as a liaison with physicians or specialists.
UNSCHEDULED DUTIES AND RESPONSIBILITIES:
Assist your supervisor with any work related tasks as requested taking initiative where possible
Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.
Support the maintenance of a safe environment by participating in training and drills as requested.
Maintain confidentiality of records relating to client’s treatment.
Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by the supervisor.
Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.
PHYSICAL DEMANDS: Regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
WORKING CONDITIONS: In clinic, in group homes.

Qualifications
COMPETENCIES AND SKILLS:
Experience: Preferably experience in both psychiatric nursing and geriatric or long-term care.
Knowledge: Expertise in psychiatric conditions and treatments, as well as chronic illness management and geriatric care.
Compassion & Interpersonal Skills: Ability to provide empathetic care to both mental health patients and elderly residents, building trusting relationships with individuals in distress or long-term care.
Organizational Skills: Capable of managing multiple care plans and addressing the needs of diverse populations efficiently.
Teamwork & Collaboration: Strong ability to work as part of interdisciplinary teams, ensuring cohesive, comprehensive care for all individuals.
LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS:
Licensing: Active RN license in Maryland or a state that participates in the Nurse Compact Agreement.
Certification: Delegating Nurse certification from a Maryland Board of Nursing approved vendor. If the candidate does not have this credential, the nurse is expected to obtain it within 30 days of starting. The company will assist will provide financial assistance to complete the course if needed.
