Job Summary: NOW OFFERING A $4,000 SIGN ON BONUS We are seeking a Site Director who will provide leadership and extensive operational and clinical support for the CCiR mobile clinic team. In this role, you will work with medical leadership to envision and implement our strategic direction: how to evolve our model of mobile addiction care, including improving the outreach model and trying new locations, with the aim of improving access to services for people who face barriers in traditional settings. Operations responsibilities will include leading monthly CCiR team meetings – setting the agenda and facilitating discussion, communicating operational and safety issues to team members in a timely manner, assuring the mobile unit is DPH compliant, ordering supplies and stocking the mobile clinic in an organized fashion, tracking and scheduling vehicular maintenance, maintaining the schedule of CCiR clinicians, harm reduction specialists, and visitors, serving as a liaison to external partners including funders (DPH BSAS and the Kraft Center for Community Health), as well as key stakeholders in the neighborhoods like the Nubian Sq Task Force and others as needed, and attending statewide mobile addiction meetings hosted by BSAS and other partners. The Site Director will supervise the Harm Reduction Outreach Specialist / Data Coordinator and the Americorps members who works with the CCiR team. Additionally, you will work alongside CCiR harm reduction specialists and other CCiR clinicians to provide high quality and equitable medical care in the outreach setting, with an emphasis on addiction medicine and primary care. Hours: Full-time, Monday – Friday 8:30am – 5:00pm. Responsibilities: Essential Duties (Managerial):

Supervision: Provide supervision for the Harm Reduction Specialist – Data Coordinator, as well as the AmeriCorps member who works with the CCiR Team, which includes participating in interviewing, onboarding, scheduling, giving feedback, teaching various direct service skills in harm reduction and case management, and providing documentation needed by the AmeriCorps program.

Community engagement: In collaboration with medical leadership, envision and implement new models of programming and new service locations to increase access to mobile addiction care.

Strategic direction: Together with medical leadership, engage with key community stakeholders in various neighborhoods to promote activities of the mobile program and build relationship in the neighborhoods.

Grants management: Draft team goals and outcomes, as requested by BSAS, as well as other grants-management activities including drafting of progress reports and budget amendments when needed. Review progress on outcomes regularly.

Quality improvement: Implement population health management and clinical quality initiatives for the mobile team, including patient registries to assist the team in improving BHCHP quality metrics, novel approaches to improve rates of vaccination, and creative workflows to involve all team members in quality improvement efforts.

Essential Duties (Operational):

Manage operational activities of van, including DPH compliance, supplies inventory and organization, and scheduling of clinical staff, harm reduction specialists, and visitors (and communicating schedule changes to AHOPE).

Lead monthly team meetings of BHCHP CCiR team; o rient/onboard new staff to the CCiR model; serve as point person to other teams within BHCHP, as well as external organizations, including AHOPE, the Kraft Center for Community Health, BPHC, and BSAS.

Attend monthly statewide mobile addiction meetings hosted by BSAS to represent BHCHP.

Prioritize the safety of CCiR team members through deliberate planning to mitigate risk, debriefing after incidents, and quickly communicating any safety concerns to team members.

Purchase equipment for mobile clinic; manage petty cash and incentive cards for CCiR van; work with Facilities, contractors, and the Team Lead to ensure the van is up to date on maintenance, including oil changes, gas, annual inspections, cleaning, and repairs.

Essential Duties (Clinical):

Serve as the clinical provider on the CCiR team as needed for 2-4 shifts per week, with support from other CCiR clinicians.

Provide urgent care and primary care for a population of people with active substance use disorder and experiencing unsheltered homelessness, which includes:

Management of chronic diseases including SUD, HCV, DM, HTN, etc.; diagnosis and treatment of sexually transmitted illnesses

Preventive care as possible, including vaccinations and cancer screenings

Urgent care, frequently including wound care; Other medical care for people who use drugs, including PrEP/PEP, complex wound care, I&D of skin abscesses

Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc.

Essential Duties (Other):

Drive CCiR van when necessary (submit driver’s license and driving record annually).

Maintain all clients' privacy and confidentiality, adhering to HIPAA regulations and those policies as implemented by the BHCHP.

Qualifications:

Candidates with lived experience are highly encouraged to apply.

Nurse Practitioner or Physician Assistant license in the state of MA required. Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency required.

Exceptional organizational skills.

At least two years of experience providing harm reduction services/addiction services with an emphasis on patients not needing to be abstinent to receive care and an openness to recovery being defined by the patient.

Experience working with people experiencing homelessness, incarceration and/or sexual violence as well as conducting street level outreach with at-risk populations is a plus.

Bilingual Spanish and English is preferred.

Life support training certification (e.g., CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to the start date, applicant must submit certification within 60 days of the start date.

Commitment to equitable and culturally appropriate care for a wide range of diverse populations, including communities of color, LGBTQIA communities, non-English speaking populations, people with substance use disorders and behavioral health challenges and people experiencing homelessness.

Current and valid Massachusetts driver’s license, with no prior motor vehicle violations record required.

Compensation and Benefits:

NOW OFFERING A $4,000 SIGN ON BONUS

The compensation starts at $96,000 and increases based on years of experience.

BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
